Online forms

There are many forms used in the day to day workings of the Auxiliary. These forms are subject to change on a continual basis. Rather than host the forms here and risk a member submitting an obsolete form, this page will link you to their location on the National website.

National Forms Site

All forms are in Adobe Acrobat format, Most computers have Adobe Acrobat reader installed, but in case you do not, click on the icon below to download the latest version for your computer.

Get Adobe Reader

There are three types of forms. Plain forms, Fillable forms, and Electronic forms. The plain forms can be printed out on your computer, and filled out manually for submission by mail or directly to the appropriate officer. The fillable forms can be filled out on your computer, printed and submitted as above. The Electronic forms can also be filled out with the computer, but then can be submitted electronically. No paper needed. More details below.

The first link will take you to the "regular" forms. These forms can be either Plain or Fillable. The trend is towards the fillable format, as it makes the form much easier to read for the officer responsible for getting the information into Aux DATA. The Fillable forms can be identified - usually - by a colored button near the top that will let you clear the form. Sometimes if you have an older version of Acrobat Reader (also called Adobe Reader), you may not be able to fill out a fillable form. Use the icon above to update your Reader. Further instructions are available at the forms link.

Regular Forms

The next link takes you to the electronic forms. Some things are required on your computer before you can use these forms. If you use a public terminal (library, school, etc.) we do not recommend the electronic forms.

First, please make sure you have the latest version of Adobe Reader (see link above). At this writing (Jan 2004), the latest version is 6. Almost all Adobe PDF files are compatible with the latest reader, so unless you really need an earlier version, update! Next, you need to be connected to the internet when you hit the "submit" button on the form. You can fill out the form while not being online, but the submission process requires the connection.

One last note: make sure you consult with the Elected or Staff officer that you submit the form to before submitting any electronic forms to them. VFC Rob Silano has already indicated that he would like to have forms submitted eletronically for those that are submitted to him.

Electronic Forms

When you click on a form at one of the websites, it is downloaded to your computer. On some systems, the file will be in a download folder, on others it will be in a temporary folder. If the form appears inside your web browser, it is almost certainly in a temporary folder. In order for you to fill out a fillable form, you must have the form downloaded to your computer and not in a temporary folder. To double check, when the form opens, save it in a place you will recall where it is later.

If you use the fillable or electronic forms, Adobe Reader will not allow you to save the filled out form. The only record you can make for yourself is a printed copy (on some computers, you can make a non-fillable PDF file when you print e.g. Mac OS X). In order to save a filled out form such that you can add to it later (useful for Member Activity Logs), you must have one of the paid versions of Adobe Acrobat software. AUXCEN has a special program where Auxiliarists qualify for government pricing on Adobe software.

Note to all: With some operating systems, you can get around this limitation by filling out the form, and printing to a PDF file. Mac OS X has this, and some Windows machines do also.

Note to Mac users: The Mac OS X application "Preview" can open most of the forms, and print them, but cannot fill out the fillable and electronic forms. Get Adobe Reader and make sure that Reader is opened for any PDF files.
